Reduced physical capacity
Reduced strength, conditioning, mobility, balance, confidence or tolerance for everyday activity.
For Healthcare Referrers
Longevico provides founder-led Exercise Physiology across Brisbane for patients who require an individualised and clinically considered approach to exercise, physical capacity and long-term function.
The service is designed to complement existing medical and allied health care, with clear scope boundaries and communication where appropriate and with patient consent.
Referral suitability
A referral may be useful when a patient needs more than general advice to begin, resume or progress exercise appropriately.
Reduced strength, conditioning, mobility, balance, confidence or tolerance for everyday activity.
Exercise participation complicated by medical history, medication considerations, symptoms, pain or previous injury.
Rebuilding physical capacity following illness, injury, surgery, treatment or a prolonged period of inactivity, once clinically appropriate.
Exercise support for cardiometabolic, musculoskeletal or other chronic health considerations within Exercise Physiology scope.
Strength, balance, mobility and conditioning support for patients seeking to maintain function and independence over time.
Patients who have received recommendations but remain uncertain about what exercise to perform, where to begin or how to progress.
Returning to work, travel, recreation, family responsibilities, sport or other personally meaningful activities.
Suitability is considered individually. In some cases, further medical review, physiotherapy, specialist input or another service may be more appropriate before Exercise Physiology begins.
The referral pathway
The referrer or patient makes contact with basic information about the reason for referral and the support being considered. Detailed clinical information should not be submitted through the public enquiry form.
Longevico considers the general request, service location, clinical scope and whether an Initial Health Consult appears appropriate.
Relevant health history, injury background, current concerns, goals, precautions, exercise history and available clinical recommendations are reviewed.
Current capacity and exercise priorities are assessed where appropriate. A suitable exercise plan, further assessment pathway or alternative recommendation is then established.
Support may include supervised sessions, individualised programming, progression, monitoring and practical guidance for implementing the plan.
With patient consent, relevant assessment findings, exercise priorities, progress or emerging concerns may be communicated to the referring practitioner.
Clinical standards
The same practitioner remains familiar with the patient’s history, exercise plan, response and functional goals over time.
Exercise decisions account for relevant history, symptoms, medications, current capacity, previous recommendations and practical barriers.
Exercise selection and dosage are matched to the patient’s presentation rather than based on a standardised program or diagnosis alone.
Clinically useful information can be communicated to the broader care team where appropriate and with consent.
Concerns requiring diagnosis, medication review, imaging interpretation, acute treatment or specialist input are directed to the appropriate practitioner.
Appointments may be delivered in a suitable home, home gym, apartment gym, approved private gym or other appropriate setting by arrangement.
Communication with the treating team
Communication is guided by clinical relevance, the patient’s consent and the reason for referral.
Depending on the situation, communication may include:
Longevico does not provide medical diagnosis, medication management or specialist interpretation. Communication is focused on Exercise Physiology findings, exercise tolerance, physical capacity, functional progress and matters relevant to safe participation.
Scope and collaboration
Longevico supports exercise prescription, physical capacity, strength, conditioning, mobility, balance, movement tolerance and long-term function within Exercise Physiology scope.
Medical diagnosis, medication decisions, imaging interpretation, acute injury management, manual treatment and specialist medical care remain with the appropriate practitioner.
Where another provider’s input is required, Longevico supports referral, collaboration or return to the treating team rather than extending beyond scope.

Private delivery
Mobile and private appointments can reduce some of the practical and environmental barriers that prevent patients from engaging with exercise.
Depending on suitability, sessions may occur in the patient’s home, home gym, apartment gym, an approved private gym or another appropriate location by arrangement.
The mobile model does not remove the need for clinical suitability. The environment, available equipment, privacy and safety are considered before ongoing sessions begin.
Making contact
Healthcare professionals can submit a brief referral enquiry using the Longevico enquiry form. Please provide only the basic reason for referral, your contact details and the patient’s suburb.
Do not include detailed health information in the public form. If the referral appears suitable, the appropriate process for exchanging referrals and relevant clinical documents can then be arranged.
