What we collect
Depending on how you interact with Longevico, we may collect information including:
your name, date of birth and contact details
health and medical information relevant to your care
medical history, medications, injuries and health conditions
exercise, training and lifestyle information
referral letters, reports and information from other health professionals
assessment results, clinical notes and exercise program information
Medicare, DVA, private health, WorkCover or other funding information where relevant
booking, payment and administrative information.
We generally collect this information directly from you. Where appropriate, we may also receive information from a referring practitioner, another health professional, a parent or guardian, funding body or another person involved in your care.
Health guide requests
The Health guides form asks only for your email address. Netlify, our website form provider, stores it with the submission timestamp, the form source and a record of the follow-up wording and permission you agreed to when submitting. These records are available to authorised Longevico staff in the restricted submission system. The existing internal form notification alerts Longevico to a new request.
After a successful request, all three guides are available to download on the website. Longevico may personally email you about the guides and how its services may help, as stated beside the form. The request does not sign you up to a newsletter or an automated email sequence, and the website does not email the guides to you.
You can opt out of guide-related follow-up at any time by replying to the email or contacting admin@longevico.com.au. Longevico keeps a restricted record of that choice so it can be respected. This permission is separate from optional-cookie consent; declining optional cookies does not prevent you requesting or downloading the guides. Contact details and form responses are not sent to website analytics.
Why we collect your information
We collect personal and health information so we can:
provide safe and appropriate Exercise Physiology services
understand your health, goals, physical capacity and relevant risks
assess, plan, deliver and review your care
communicate with you about appointments and your services
coordinate with other health professionals involved in your care where appropriate
process payments, rebates, referrals and funding requirements
maintain appropriate clinical and business records
meet our professional, legal and regulatory obligations.
If we do not receive information that is reasonably necessary for your care, we may not be able to safely or appropriately provide some services.
Access, correction and privacy concerns
You may request access to, or correction of, personal information we hold about you.
